To open the form you need:

1. Most forms will open in your Internet browser or your word processor.
2. MS Word files are READ-ONLY templates.
3. PDF files (*.pdf) can be printed and then filled out. Download and use Adobe Reader to view the document. Acrobat Pro users can save the data.
4. Excel spreadsheets (*.xls) might require MS Excel to open.
5. Once you make a selection, you can "Open" or "Save" the document.
